New Disability Tax Credit Application form
Effective Oct. 4th, 2021
The CRA has produced a new form for all future DTC applications. If your doctor has not signed your forms prior to Oct. 4, 2021, we suggest connecting with an advocate to discuss the changes and how they can impact your application.
The new form is longer and more detailed and your doctor may not have even seen the new form yet. If you would like to review the new form with our advocates we are happy and ready to assist you.

Here is the new Disability Tax Credit application form. It has changed from a 5 page application to a 15 page application.
